For this session, use the preceding feature on supporting older people, as well as other sources of information, to strengthen your knowledge. This will give you more insight and information to pass on to your customers. Take time to think and chat about what contribution you can make to the health of your older customers. Consider the following:
- How can the pharmacy team help people who have been transferred from hospital back to the community – either home or to another care setting?
- What medication services can community pharmacy provide for patients discharged from hospital? Does your pharmacy provide any of these already?
- Does your pharmacy provide services to care homes or domiciliary visits? Does each member of the team understand what these services involve?
- What are the five main causes of premature death and how can older customers reduce their risk of these?
- Which vitamins and minerals might older people lack and why? Which supplements do you stock in the pharmacy that are formulated for older people?
