What matters most to the patient?
The purpose of the NHS Discharge Medicines Service consultation with the patient and/or carer is to check their understanding of which medicines they should be using post-discharge, when they should be used, and to provide any other advice the patient/carer requires to support safe and effective medicines use.
There could be many different issues to consider, and it is important to find out what matters most to the patient. By using effective consultation skills and applying a structured information-gathering approach, pharmacy professionals can work out how best to offer the support someone needs and wants.
