NHS Discharge Medicines Service: part six

Welcome to the sixth module in our series looking at the NHS DMS. It is based on the Centre for Pharmacy Postgraduate Education (CPPE) NHS Discharge Medicines Service: improving outcomes for patients transferring care e-learning programme, and is designed to give pharmacy technicians an overview and better understanding of this area of practice.

Learning Objectives

After reading this module, pharmacy technicians will:

• Understand the importance of person-centred care and shared decision making
• Be able to apply these principles to consultations as part of the NHS Discharge Medicines Service
• Know how to use the LICEF tool to gather information.
