Stage two

  • The second stage of the service occurs when the first post-discharge prescription is received, or when the patient or their carer makes first contact after discharge. Note that the three stages of the service may occur in parallel and first contact with the patient could happen at any stage
  • The pharmacist or pharmacy technician will ensure that medicines prescribed post-discharge take account of any appropriate changes made during hospital admission. If there are any discrepancies, these should be resolved with the prescriber. This is usually the GP, but this could also be a practice-based/PCN pharmacist or a specialist clinic or nursing team. In some cases there may need to be further liaison with the hospital or NHS Trust team
  • Stage 2 of the service occurs when the first post-discharge prescription is received. This is usually one week to one month post-discharge, but there is no defined time limit
  • At each stage of the service, the community pharmacist or pharmacy technician should make a note of actions taken and relevant information on the PMR or other record.
