  • Here, the community pharmacist or pharmacy technician will have a confidential discussion with the patient (and/or their carer if appropriate) to check their understanding of their medicines regimen and offer additional support if required. Patients with complex issues who may benefit from a Structured Medication Review should be offered a referral to their GP practice or PCN pharmacist
  • Where the patient and/or their carer cannot attend the pharmacy to discuss their medicines, this support and advice can be provided in a manner that meets their needs, e.g. by telephone or video consultation
  • Following consultation with the patient at stage three of the service, relevant information to support ongoing care will be communicated to the GP practice or PCN pharmacist, if it is clinically appropriate. The patient’s consent must be obtained, and the information transmitted using a secure method
  • At each stage of the service, the community pharmacist or pharmacy technician should make a note of actions taken and relevant information on the PMR or other record.
