  • Start at the top: a commitment to safety is needed from senior management and so it needs their support
  • Make it clear to everyone that safety matters: it needs to involve the whole workforce and everyone ought to get involved
  • Encourage learning from errors: this involves developing an error reporting and learning system, as opposed to just reacting to, or counting, errors
  • Search for solutions: this includes making use of risk assessment methods – such as FMEA and PRIMO – which will be discussed in a later part of this module
  • Prepare people to work safely: this includes developing and evaluating training programmes.
