Accessing a patient’s record is done on a case-by-case basis; the exact procedure will be determined locally and is also software dependant. In general terms the process is as follows:
- Patient reports to pharmacy
- The pharmacy professional searches for the patient on the SCR system
- Once the patient is located they click view
- The pharmacy professional then asks the patient for permission to view
- The patient grants or denies permission to view
- The pharmacy professional records the patient’s answer by clicking yes or no
- The pharmacy professional should also enter the reason for accessing the SCR in the free text “enter reason for access” box. For example, “providing MUR”
- If permission is granted the pharmacy professional is then looking at the SCR
- If a pharmacy professional decides that he or she needs emergency access to the patient’s SCR as it is in the patient’s best interest, they select the emergency access option and enter the reason into the free text box and the SCR is displayed.
