For sprains and strains, the NICE Clinical Knowledge Summary (CKS) recommends paracetamol or a topical NSAID, such as ibuprofen gel. Codeine can be used in addition to paracetamol if necessary, but wait 48 hours before considering whether an oral NSAID is needed.

Last year, Cochrane reviewers updated their review of topical NSAIDs for acute musculoskeletal pain in adults,7 and found further studies adding to the evidence that "topical NSAIDs are effective in providing pain relief". In addition, "certain formulations, mainly gel formulations of diclofenac, ibuprofen and ketoprofen, provide the best results".

Liniments and embrocations are useful to help with sprains and strains as warm or cold therapy.
