General consultation points

The details and outcome of each consultation must be recorded on the same day on an NHS assured Pharmacy First IT system. Provision of the service should be notified to the patient’s GP on the same or following working day.

With the patient’s consent, their GP record or their National Care Record must be consulted unless there is good reason not to do so. (Note: Use of GP Connect requires contractors to agree to the terms of the National Data Sharing Arrangement). Where an action is required by the GP team (such as booking the patient in for a follow-up appointment) an URGENT ACTION communication must be sent to the practice. 

The patient must be advised that information sharing will take place with NHS England as part of the service monitoring, and with the NHSBSA and NHS England for the purpose of contract management. Verbal consent to receive the service must be sought from the patient and recorded in the pharmacy’s clinical record. 

When NHS medicines are supplied, an NHS prescription charge per item should be collected (if applicable). Charges collected from patients will be deducted from the sum payable to the pharmacy. 

A blank FP10DT EPS dispensing token must be used to record any medicines provided to patients exempt from prescription charges. This must include: 

  • Patient’s full name, address, date of birth and NHS number 
  • Name, strength and form of medicines requested, and quantity supplied 
  • Date of supply 
  • Name and address of patient’s GP 
  • Referral ID number (if the patient was referred).
