NHS Discharge Medicines Service: part three

Welcome to the third module in our series looking at the NHS DMS. It is based on the Centre for Pharmacy Postgraduate Education (CPPE) NHS Discharge Medicines Service: improving outcomes for patients transferring care e-learning programme, and is designed to give pharmacy technicians an overview and better understanding of this area of practice.

• Understand what hospital and acute care teams are responsible for in the NHS DMS
• Be aware of the situations deemed high-risk where an NHS DMS referral would be of benefit
• Know what kind of information should be transferred between settings.
